Ziel der Studie war es die phänotypischen Eigenschaften von Immunzellen im venösen Blut von Nierenzellkarzinompatienten unter Anwendung durchflusszytometrischer Analysemethodik zu charakterisieren. Zudem wurde die immunsuppressive Wirkung der chirurgischen Intervention in Bezug auf das gewählte Operationsverfahren (laparoskopisch vs. konventionell-offen) untersucht. Tumorpatienten wiesen im Vergleich zu gesunden Kontrollpersonen eine höhere Granulozyten/DC-Ratio auf. Charakteristisch für ein fortgeschrittenes Tumorstadium war eine verminderte Anzahl an DC eine erhöhte Granulozyten/DC-Ratio sowie eine verminderte monozytäre HLA-DR-Expression. Laparoskopisch operierte Patienten wiesen postoperativ einen moderateren Abfall der monozytären HLA-DR-Expression auf. Die Resultate zeigen dass die Analyse der Leukozytensubpopulationen ein perioperatives Immunmonitoring ermöglicht. Die monozytäre HLA-DR-Expression ist ein geeigneter immunologischer Marker zur Überwachung der perioperativen Immunsuppression.

The aim of the study was the identification of surrogate markers of host immunity in RCC patients using multi-color flow cytometry. Furthermore the time series of these immune markers of RCC patients undergoing either open surgery or laparoscopy was compared. Tumor patients showed higher values of the granulocyte/DC ratio than healthy individuals. Tumor progression was associated with a reduced frequency of blood DCs higher values of the granulocyte/DC ratio and a reduced HLA-DR surface expression. Tumor surgery caused a decrease of monocytic HLA-DR intensity. We demonstrated by time kinetic analysis that laparoscopic intervention caused a more moderate immunsuppression. These results suggest that the phenotype of innate immune cells reflects well the differences in the cellular immunity of RCC patients associated with tumor disease as well as surgery. The monocytic HLA-DR intensity represents a suitable marker for monitoring tumor stage and surgery-associated immunsuppression in RCC patients.
