Einleitung und Fragestellung: Klinische Studien konnten zeigen, dass eine glutaminhaltige Ernährung die Erkrankungsschwere und Liegedauer von Intensivpatienten senken kann. Als Infusion liegt Glutamin jedoch immer als Dipeptid vor, meistens in Kombination mit Alanin. Ziel dieser Arbeit war herauszufinden, ob 1. Glutamin und 2. Alanin für sich getrennt und schließlich 3. auch die Kombination dieser beiden Aminosäuren (Alanyl-Glutamin) in-vitro einen Effekt auf die Zytokinexpression von humanen Monozyten unter septischen Bedingungen haben. Material und Methoden: Blutproben (200µµl) von gesunden Probanden wurden 1. mit 2mM und 5mM Glutamin (n=12), 2. mit 2mM und 5mM Alanin (n=13) und 3. mit 2mM und 5mM Alanyl-Glutamin (n=12) versetzt. Nach LPS Stimulation (0,2ng/ml) wurden die Proben für 3 Stunden bei 37°C inkubiert und anschließend die intrazelluläre Zytokinexpression von IL-6, IL-8 und TNF-α durchflußzytometrisch bestimmt. Zum Ausschluß eines toxischen Effektes der untersuchten Substanzen auf die Monozyten wurde eine 7AAD Messung durchgeführt. Die Ergebnisse wurden anhand des Wilkoxon Test ausgewertet, das Signifikanzniveau anchliessend nach Bonferroni korrigiert. Ergebnisse: Nach LPS-Stimulation reduziert Glutamin signifikant die IL-6 und TNF-α Expression. IL-6 wird durch eine Glutaminkonzentration von 2mM um 20% (p Alanin verringert unter LPS Gabe ebenfalls signifikant die TNF-α Produktion. Diese wird durch 2mM Alanin um 18% (p Diskussion: Die Ergebnisse der Studie zeigen, dass sowohl Glutamin als auch Alanin in-vitro einen Effekt auf LPS-stimulierte Monozyten haben, der beim IL-6 entgegengesetzt, beim TNF-α aber gleichgerichtet ist. Die Wirkungen addieren sich beim Alanyl-Glutamin, so dass hier keine IL-6 Veränderungen, jedoch eine Reduktion der TNF-α Produktion beobachtet wird. Da in der hyper-inflammatorischen Phase der Sepsis eine Reduktion möglichst vieler Zytokine von Vorteil sein kann, schließen wir aus unseren Ergebnissen, dass Alanin den erwünschten immun-modulatorischen Effekt des Glutamins überdeckt.

Background: Clinical trials demonstrate that glutamine-enriched nutrition may reduce morbidity and the length of stay in intensive care patients. Glutamine, as a single substance, is very instable in aqueous solution; consequently it is administered as a dipeptide, mostly in combination with alanine. Our aim was to investigate whether 1. Glutamine and 2. Alanine seperately and finally 3. the combination of both amino acids (Alanyl-Glutamine) have an effect on the invitro cytokine expression of humane monocytes under septic conditions. Methods: Blood samples (200µl) of healthy test persons were incubated with 1. 2mM and 5mM Glutamine (n=12), 2. 2mM and 5mM Alanine (n=13) and 3. 2mM and 5mM Alanyl-Glutamine (n=12). LPS (0,2 ng/ml) was added for stimulation and the samples were incubated for 3 hours at 37°C. The intracellular cytokine expression of IL-6, IL-8 and TNF-α was determined by flow cytometry. To exclude toxic effects of the investigated substances on humane monocytes, testing with 7AAD was performed. Results: Glutamine significantly reduces IL-6 and TNF-α production in LPS stimulated monocytes. While glutamine reduces IL-6 production, it is enhanced by alanine. Both substances decrease intracellular TNF-α production. The effects of the single substances add up in their combination Alanyl-Glutamine. Here, no significant change in IL-6 Production is seen, whereas TNF-α is diminished. Because the reduction of a maximum of proinflammatory cytokines during hyperinflammatory phase in sepsis may be of great benefit to the patient, we conclude that alanine masks the immunmodulatory effect of glutamine.
