Angesichts der stetig wachsenden Zahl der Endoprothesen-Implantationen an Hüft- und Kniegelenken und der erhöhten Lebenserwartung, steigt auch die Inzidenz postoperativer periprothetischer Femurfrakturen. Zu den Risikofaktoren zählen neben der generalisierten Osteoporose, Prothesenlockerungen, und Revisionseingriffe, wobei in den meisten Fällen ein Bagatelltraumata für die Fraktur verantwortlich ist. Die erfolgreiche operative Versorgung dieser Frakturen erfordert eine sorgfältige Therapieplanung. Neben der konventionellen und minimalinvasiven Frakturstabilisierung, steht der Prothesenwechsel als Therapieoption zur Verfügung. Die Wahl des Therapieverfahrens ist neben der Frakturlokalisation und Prothesenstabilität auch abhängig von der Knochenqualität und dem Allgemeinzustand der Patienten. Anhand einer retrospektiven klinischen Untersuchung von 46 Patienten mit postoperativer periprothetischer Femurfraktur, die zwischen 1990 und 2002 operativ behandelt wurden, sollen sinnvolle Therapiekonzepte vorgestellt werden.

In view of the rising number of hip and knee joint replacements and the increasing life expectancy, also the incidence of postoperative periprosthetic femur fractures is rising. Risk factors are as well generalized osteoporosis as loosening of the prosthesis and revision arthroplasty. In most cases minimal trauma is causal for the fracture. A successful surgical treatment requires a careful preoperative planning. Therapeutic possibilities are conventional and minimally invasive fracture stabilization and change of the prosthesis. The decision for the right treatment depends on the location of the fracture, the stability of the implant and also on the quality of bone and the patient’s general state of health. The aim of this retrospective clinical study of 46 patients with postoperative periprosthetic femur fractures, treated between 1990 and 2002, is, to present useful therapeutic procedures.
