In dieser prospektiven Studie wurden 34 Patienten 3 und 12 Monate postoperativ untersucht. Ziel der Studie war es die Entwicklung der funktionellen Wiederherstellung zu erfassen und das Gesamtergebnis zu dokumentieren. Mögliche Faktoren die das Gesamtergebnis beeinflussen werden erfasst. Wir wendeten den Constant- Neer- und DASH-Score an. Die Pflegebedürftigkeit wurde mittels Barthel-Score ermittelt. Außerdem wurden nach 3 und 12 Monaten Röntgenaufnahmen angefertigt und ausgewertet. Zwischen 1. und 2. Untersuchung konnte ein signifikanter Unterschied im Constant- und DASH-Score ermittelt werden. Hinweise auf wesentliche beeinflussende Faktoren konnten nicht gefunden werden. In Zusammenschau der vorliegenden Ergebnisse und der Literatur lässt sich feststellen dass beim älteren Patienten trotz eingeschränkter Funktionsfähigkeit auf Grund der meist bestehenden Zufriedenheit und Schmerzfreiheit eine Prothesenimplantation bei mehrfragmentären Humeruskopffrakturen favorisiert werden sollte.

In this prospective study 34 patients were examined both 3 and 12 months after their surgery. The goal of this study was to record the development of the functional rehabilitation and to document the final result. Factors that could possibly influence the end result are being included. We used the Constant- Neer- and DASH-Score. The dependency on care was calculated via the Barthel-Score. Additionally x-rays were taken and analyzed both 3 and 12 months after the surgery. Between the first and the second examination a significant difference was found in the Constant- and DASH-Score. There was no evidence of significant influential factors. Summarily the documented results and the available literature lead to the conclusion that in spite of limited functionality prosthesis implantation should be favored in case of multi-fragmented humeral head fractures due to the usually resulting satisfaction and absence of pain.
