Die CIS20-R wurde bei ALS benutzt um physische und mentale (Konzentration Motivation Aktivität) Fatigue zu messen die 3/s-RNS zur Beurteilung der peripheren Fatigue (Dekrement des Muskelsummenaktionspotentials (MSAP). 51 Patienten wurden prospektiv untersucht. CIS20-R FSS die Dekrementhöhe und die Abnahme Amplitudenabnahme des MSAP wurden mit klinischen ALS-FRS-R MRC Score und slowVC verglichen. Fatigue zeigte sich zum Einschluss bei 59% (FSS) bei 42% (CIS20-R). FSS und CIS20-R (außer: Konzentration) zeigten einen kontinuierlichen Anstieg im Verlauf. Mit Erkrankungsdauer tritt eine Allgemeinzustandsverschlechterung ein (Fatigue Aktivitätsverlust Motivationsabnahme). Die Amplitude des MSAP war unabhängig vom Vorhandensein eines Dekrementes keine Korrelation bzgl. Dekrementhöhe/Amplitudenabnahme/klinischen/Progressionsparametern/F atigue. CIS20-R ist sensitiv um Fatigue in frühen Krankheitsstadien zu erfassen. 3/s-RNS hat keinen prognostischen Wert. | |
Fatigue may be an underestimated symptom in ALS. The CIS20-R (subjective fatigue experience/concentration/motivation/activity) was used to measure both physical and mental fatigue. The decrement of the CMAP in RNS might be a measure of peripheral fatigue. 51 consecutive patients were assessed up to 12 months scores were compared with ALS-FRS-R/MRC score/slowVC. Fatigue was seen in 59 % (FSS) and 42 % (CIS20-R) (baseline) steadily increasing (except: concentration). A pathological decrement occurred from 2.3% (hypothenar) to 14.3% (trapezius) (baseline). After 12 months 27% of the remaining 18 patients had a pathological decrement (trapezius). No correlation between CMAP/decremental response with functional/progression parameters/fatigue scores. CIS20-R is a sensitive tool to detect fatigue in early stages of ALS. A pathological decrement is seen more frequently in recordings of nerves with low CMAP (especially trapezius muscle). RNS has no prognostic value. |
